The DaySpring forms are PDF files, which require Adobe Reader. 
 If not already installed on your computer, download it free from the link below.

These forms have been successfully tested on Adobe Reader versions 5, 6 and 7. 
 If you experience problems with an older version, try upgrading to the latest version.  

For some reason known only to the Cyber Guardians 
-- who have a divine appointment to puzzle and befuddle (and humble) the average user --
 there are times when a request for a pdf form will result in a blank page with no form. 
 My experience has been that this Cyber Nonsense is cured by rebooting the computer.  
Voila.

The information packet you need when enrolling in DaySpring Academy.    


The 4 forms needed to apply for enrollment with DaySpring.   Each form should print as one page only.  The CSEF and the RSR can not be used if formatted incorrectly and not fully contained on one page.    If you have problems printing any of these forms, please e-mail dayspringacademy@att.net or try the Word formatted files.

Re-Enrollment Form -- revised 5/14/06 Required yearly of all re-enrolling families, regardless of number of years of enrollment.  Due by August 31st  for best pricing.

Enrollment / Exclusion Form  --
Added 5/11/06		 Form DL-1/93 -- This is the form needed when church school students apply for a driver's permit / license.  Instructions for completion of applicant section are included.  Mail completed form to Donna Knox for the required verification of current enrollment; include SASE for return of form.
